LATEST INFO from City of Hollywood Emergency Management Director Jaime Hernandez and Marie Woodson, our State Representative. A BIG THANK YOU to both of them for making this happen.
The registration form will be available in English, Spanish and Creole. Unfortunately, people will not be able to choose a preferred time slot. Time slot will be given to them randomly. This round of vaccines are for OVER-65 only.
Each building needs to gather the following info. You will need volunteers in your building to compile this info and hold on to it. Once we are ready, the volunteers will manually input the information into the form. This way there is no worry about who has a computer and who does not. This team of volunteers may also be asked to assist the state during the vaccination event.

Cindy Abraham will be the point person that will coordinate all this with the state strike team that will administer the vaccine. Rep. Woodson will have someone from her staff show your volunteers how to access and print the information on the form. The state is looking to set this up within the next two weeks so we need to get moving on this ASAP. Rep. Woodson is working with the state to have this site open for multiple days to serve our Hillcrest residents. RESIDENTS WILL NEED TO BRING THEIR TO THE VACCINE SITE to confirm their age!
